Each patient is assigned their own personal physical therapist who utilizes one -on-one “hands on” physical therapy and therapeutic exercise based on the newest research to achieve the best results in fewer visits offering our patients a significant savings. In order to begin treatment and expedite your therapist’s initial evaluation, we would appreciate it if you would take a few minutes to read over, print and fill out the appropriate forms below. Please bring the completed forms with you to your first appointment.
