Getting Started

Getting started with Westwood Physical Therapy is as easy as ONE-TWO-THREE:

1. Make an Appointment

– Call our office to schedule an appointment at 1.310.996.0085, or send an email to rehab@westwoodpt.com.

2. Gather Your Insurance Information   

– Have your insurance information ready when you call or email us:

 -Name of insurance company. Check our list to see if we have a contract with your insurance company >> Insurance Companies

3. Doctor’s Prescription

-Have your doctor’s prescription available. This will include the name of your referring physician and his/her phone number. If you haven’t seen your doctor yet, feel free to contact us and we will assist you in this process.

THREE simple steps and you’re done! If you’ve already completed the above steps, and have a scheduled appointment, just download the initial forms, print and fill them out, and bring them with you to your appointment!
