What to Bring

Along with the forms you print and fill out, please bring the following:

• Your insurance card.
• Driver’s license or other picture ID.
• Comfortable clothing and shoes. You may change here in our office if necessary.
• If you have a deductible or co-pay, please be prepared to make a payment on your first visit. We accept Visa and MasterCard, personal checks and cash. If you have not yet spoken with the front desk about your insurance benefits, please be sure to call us with your insurance information before your first visit. We verify your benefits as a courtesy to you so that you are aware of your financial responsibility. Thanks!
